Here’s what you need to know about Medicare hospice care.
What is Hospice Care?
Hospice care focuses on comfort rather than cure. It’s intended for people with a terminal illness and a life expectancy of six months or less if the disease runs its normal course. The goal is to improve quality of life, not to treat the illness aggressively or prolong life at all costs.
Hospice services include:
- Pain and symptom management
- Emotional and spiritual support
- Medical equipment and supplies
- Medications for symptom control
- Counseling and grief support for family members
- Short-term inpatient care when needed
- Respite care for caregivers
Most hospice care happens in the patient’s home, but it can also be provided in a hospice center, nursing home, or hospital.
Who Is Eligible for Medicare Hospice Benefits?
To qualify for Medicare-covered hospice care, you must meet all of the following conditions:
- You have Medicare Part A (Hospital Insurance).
- Your doctor and the hospice medical director certify that you have a terminal illness with a life expectancy of six months or less.
- You agree to receive comfort care (also called palliative care) instead of care to cure your illness.
- You sign a statement choosing hospice care rather than other Medicare-covered treatments for your condition.
It’s important to note that you can stop hospice care at any time and resume curative treatments if you choose. You may also switch hospice providers once during each benefit period if needed.

What Hospice Services Does Medicare Cover?
Medicare covers a wide range of hospice services at little or no cost to you. These include:
- Doctor and nursing services
- Pain relief and symptom management medications
- Durable medical equipment (like hospital beds or wheelchairs)
- Medical supplies (such as bandages and catheters)
- Aide and homemaker services
- Physical and occupational therapy, if needed, for comfort
- Social work services
- Dietary counseling
- Spiritual and grief counseling
- Short-term inpatient care for pain or symptom crisis
- Respite care (up to 5 days at a time to give caregivers a break)
Hospice care also includes support for family members, including bereavement counseling for up to 13 months after the patient’s passing.
What Medicare Hospice Doesn’t Cover
While hospice care is comprehensive, there are a few things that Medicare does not cover:
- Treatment intended to cure your terminal illness
- Care from a provider that isn’t arranged by your hospice team
- Room and board, unless you’re receiving inpatient care or respite care
Also, if you need emergency care unrelated to your terminal illness, Medicare may still cover those services under other parts of your plan.
What Will You Pay Out of Pocket for Hospice Care?
In most cases, you won’t pay anything for hospice services under Medicare.; however, there are only a few minor out-of-pocket costs:
- A copayment of up to $5 per prescription for pain or symptom control medications.
- 5% of the Medicare-approved amount for inpatient respite care.
Compared to other types of medical treatment, hospice care is typically much more affordable for families, and the emotional and logistical support it provides is often invaluable.
